Ok If you want your PUK code, you will have to call your service provider (etc. optus, virgin mobile) and they will transfer you to a specific number which will give you the information you need, then you will have to state your name and date of birth and then they will give you your PUK number
You need to enter your PUK code only if you entered the PIN code wrongly for the third time. PUK codes can only be retrieved from your service provider via their websites/hotline.
