The difference between them is that in a chemical reaction, the chemical make up of the substance put into the experiment is changed or modified. In a physical change, only the physical attributes of the substance is changed, like size, shape, color, density, etc.

Chemical reaction is when two or more chemicals combine and react to change (into one single chemical, two different chemicals that are not identical as the original or into more chemicals). Physical change refers to changes that are visible or can be sensed (usually touch for temperature change or simply color/state change).
