"Fancy" used to be a required term to indicate that the food in a can or jar had been processed (like peeling and coring) or had been flavored (like with the addition of seasoning or spices) It is a throw-back from when canned commercial foods first became available. When people bought a can of apples, they expected there to be whole apples in there, seeds, peel and all. So to designate foods that had been prepared in some way, like peeling and coring them - they were required to use the word "Fancy." In ketchup - the word "fancy" just means seasoning has been added to it and it isn't just sweetened tomato sauce in there.
