When I was a forecaster, the terms had a dual meaning - current coverage as well as the amount of an area that would be covered at least at some point during a specific time period. That definition might still exist, but I need to get some sleep so this is all you get. ;-)

Isolated means that there is an isolated front, so the thunderstorm is in one spot. Scattered is any kind of storm, no isolated front.

It's just a matter of how likely they are to occur. Isolated refers to around a 20% chance, while scattered I believe covers the range of 40-60% or so.
