A phobia is a fear of a specific thing or situation (although there is the condition called panophobia, which means fear of EVERYTHING). While it can be highly irrational, the sufferer's view of the world is otherwise unaffected; in fact, a phobia sufferer is usually aware of the irrationality of their own phobia(s).

Paranoia, on the other hand, is a generally unrelated fear of persecution, of being personally and specifically targeted by something or someone. And one of its symptoms is that the sufferer is often unable to see or accept the irrationality of such a state. S/he believes to be right and the others to be wrong.
