Substernal and retrosternal mean the same thing,: its an adjective describing the anatomical area below, beneath or behind the breast bone. Infrasternal is another term meaning the same thing but used less often. Pain there can indicate numerous different conditions from cardiac issues to swallowing problems caused by a yeast infection in the throat, hiatal hernias or esophageal spasms, to name a few. Only a dr. can tell you which one it is after exam and tests.
Retrostenal pain refers to pain that is in the middle of the front of the chest. Since both the heart and esophagus are in this area, the pain is often associated with acid reflux and called heartburn, but usually has nothing to do with the heart. If you are also experiencing breathing difficulties, sweating, of radiating pain, then you may be having a heart attack. In any case, if you are concerned, call 911.

Well if you have a 'high pain threshold' you seem to feel a lot more pain then you do if you have a 'low pain treshol'.
