An evaluation is done when a job is already finished to critique work you have already done, or your overall performance of a job over time, for example. A yearly evaluation of your overall job performance might be done once a year to determine if you get a raise, get promoted or demoted, etc, and to give advice on how you might improve whatever is being evaluated with future actions.

Or an evaluation of an antique might be done by an art expert to determine what it is worth, for insurance purposes, or before it is put in an auction, or the evaluation might be to determine if it is fake or real. An evaluation is to access something physical or an event or action that has already happened.

Supervision is periodically watching someone do a job or craft or whatever they are doing, and giving critique or advice if they aren't doing it correctly right then. A supervisor is a person responsible for making sure a crew of less experienced or less responsible people are doing an adequate job, or arriving on time, or not goofing off, or staying on budget for a project, for example. Supervision involves evaluation of the situation, but it is more to make sure something is done right while it is happening, rather than to make a determination about a physical object or something that has already happened.
