According to the Catholic Church...

The philosophy regarding the bread and wine as to the presence of Jesus in space and time, the multiplicity of His body and blood, His whole and entire substance contained within the confines of the Host, and other such studies are much too in-depth and long to set forth here and require a certain amount of familiarity with certain concepts before one can grasp them. They, however, can be found set forth in theological textbooks if one is interested.

With Catholics, it is more about theology than philosophy in regard to the bread and wine, which when consecrated are believed to be the actual Body, Blood, Soul and Divinity of Jesus Christ which miraculously remain under the appearance of bread and wine. Explained philosophically, or hylemorphically, that means that a substantial change has occurred - the substance of bread and wine replaced with the substance of Christ - while the accidents of bread and wine - that is the physical qualities - remain.

The bread and wine are symbols of Jesus' body (bread) and blood (wine). This is used to physically accept Jesus into us. This symbol reflects on the passion, or death and suffering of Jesus, specifically how He laid His own life down to save us from eternal death in sin.
