Water is an easy example if it gets too hot it turn into water vapor a gas when that rises into the air it forms clouds and falls back down was water and if it gets too cold it freezes. (its a bit more technical than that but that's basically it)

I believe the term phase transition is appropriate here as each state of matter ie solid, liquid and gas, is a different phase of the same element or compound.

So for example at the point when water boils it transitions from its liquid phase to its gaseous phase.
