This answer is based around the movie as I would assume it is where the question comes from. The writer of the movie wanted to give the movie a name that would be more of a pun to compliment the odd nature of the message hidden in the film. The word 'clockwork' is a direct comparison to a person who is almost mechanically responsive. The word 'orang' (the 'e' was added for looks) is translated from the Malaysian word for 'man'. A person (or creature) who can only perform good or evil is "a clockwork orange - meaning that he has the appearance of an organism lovely with colour and juice, but is in fact only a clockwork toy to be wound up by God or the Devil; or the almighty state" - Anthony Burgess
