It really depends on the spirit, I personally have seen from as low as 35 proof (17.5%) to 190 proof (95%). It just depends on the brand, and the laws of the country/region in which it is being sold.

It depends on the type of distillation, the solvents and solutes present, their initial concentrations, and a few other things. But if you are referring to heated fractional distillation of ethanol from a mainly water based solution, then the short answer is you can get any concentration you want. The reason why you see 40% or 80proof so often is that to get higher proof liquor requires a sudden jump in energy requirements (eutectic point).

It depends on the type of liquors being made. You can guess the liqueurs will sit around usually 20-30% ABV (with the exception of Drambuie, which is 40% ABV), while the spirits will sit at above 30% ABV (usually at least 40% for whiskies and certain rums).
