It doesn't mean anything is wrong. I have a tilted cervix. Sometimes it makes becoming pregnant more difficult, but I had no problem getting pregnant. My doctor said it's normal and nothing to worry about.

If you are asking about the uterine cervix, it is common for the cervix to be very firm throughout most of the menstrual cycle. The cervix only becomes very soft and opens up when you are nearing ovulation, to make it easier for sperm to get through it to the uterus. So really it's hard most of the time and only softer a few days out of every month.

You need long fingers and short finger nails. For most women it will be jsut in front of your rectum at the bottom of your vagina, for some that have a tilted or retroverted uterus, it will be behind your pubic bone closer to your bladder.
As far as I know, if your cervix is open, a miscarriage is inevitable. I could be wrong, but that's the way it was explained to me. I've had one miscarriage in the past.
