There does not appear to be a scientific name to describe either the fear of film slides, or the fear of playground equipment (a slide).

A fear of film slides may stem from an aversion to the repetitive, loud noise the film projector makes. An aversion to looking at yourself in a slide, or a photo, may indicate a fear of yourself or how you look, or a fear of mirrors since viewing a photo of yourself is similar to viewing yourself in a mirror.

Autophobia is the fear of being alone, or oneself.

Catoptrophobia is the fear of mirrors.

Ligyrophobia is the fear of loud noises.

A fear of a slide in a playground may stem from a fear of injury, or from a fear of repeating a bad experience that took place near a slide (as in being bullied as a child). While some find the speeding motion of going down the slide exhilarating, others find it terrifying and so this could also be a possible reason for your fear.

Traumatophobia is the fear of injury.

Kinesophobia is the fear of movement or motion.
