Physicians / doctors treat medically. Surgeons do surgery. But it depends on the type and severity of injury.

As some examples:

  1. A person is in a traffic accident. It bruises his kidneys. Tests show there is no structural damage. The physician orders medications and IVs. This helps the kidneys to heal themselves while keeping the kidneys processing wastes. Nurses watch the I/O -- intake of fluids and urinary output. A frequent record is kept for the I/O. Physicians use these recordings to decide the next course of action.
  2. Three days later, the man's urine has a small amount of blood in it. More tests are done. Now a CT scan shows a blood vessel was damaged, too. Physicians write orders for a surgical consult.
  3. At first the surgeon thinks the kidney is compensating on its own. Blood work shows no abnormalities. He orders other medications and IVs. The nurses watch the man's condition more closely for any sign of deterioration.
  4. The man's urinary output changes to bright red. The nurse notifies the doctor and surgeon. Both re-examine the patient; he now has more pain, a low grade fever, and more importantly, his blood work has begun to shift into bad results. The surgeon decides to do exploratory surgery to locate and cauterize the bleeding blood vessel.

Medicine and surgery both have their roles in correcting an injury to an organ. BUT a lot goes on in deciding what to do and when. If the body can heal itself with supportive measures, that is best. But sometimes, surgery is needed.

  • Radiation or chemotherapy
  • Trauma

Often, doctors will treat kidney injuries with a procedure called dialysis. During dialysis, the patient is connected to a machine that takes over kidney function. This process speeds up the filtering of bad things and gets them out of the body faster.
