The President, as the civilian Commander in Chief, is above any general but the US doesn't have a commissioned rank above that of General.

During WWII a new 5-star rank of General of the Army was created for the top allied commanders - Eisenhower, Bradley, Arnold, Marshall, and MacArthur - because a position was needed that would allow these officers to command many other generals who were themselves already at a 4-star rank. The Navy also created a parallel rank of Fleet Admiral. These positions were equivalent to the European rank of Field Marshal. That name was considered in the US but wasn't used due to the connotations that it held at the time.
During WWI General John Pershing, as sole commander of the US forces, was given the special rank of General of the Armies (plural). No additional stars were assigned but technically it would have been a six-star rank.
About 20 years ago Congress (very) posthumously promoted George Washington to the rank of General of the Armies and declared that no other individual would hold that rank, with the intent that no future commander would be at a higher grade than the first President.

Very basically speaking, the difference is one step in rank. While a Lt. Colonel is normally referred to a Colonel, a Colonel is sometimes to referred to as a "full bird colonel" to distinguish the difference. In the Army at least, a Lt. Colonel wears a clover leaf rank insignia (silver, I think) but a full Colonel wears an eagle insignia, hence the reference "full bird."
