Military hierarchies tend to be much the same everywhere in every time: small, manageable units under a commander are placed under the command of a higher commander, and these larger groups are placed under the command of even higher commanders. The terminology and precise size of these groups changed from time to time and from place to place, so an answer could only be made about a particular army or navy in Shakespeare's time.

Othello was the commander or captain. His lieutenant (Cassio) was second-in-command. The ensign or ancient (Iago) was the commander's flag-bearer and was usually third-in-command, the equivalent of a second lieutenant. These are all of the officers we hear about.
