The "head" is the closed end of the cigar. You light the "foot". You will have better luck using a cigar cutter. But, to each his own. I saw a friend use a tooth pick to punch a hole in the end of a fine cigar the other night instead of using my $50 Xikar cutter???

Yes , a cigar may be smoked til it's down to the nub but usually the cigar will go "hot" because it's so close to the end .
A vintage cigar cutter works by punching a small hole in the end of the cigar one has in their mouth. One simply the cutter on the cap of the cigar and trigger the punch. It is different from the more modern guillotine method.
