A hole or a crack in the exhaust or muffler.
Try checking the transmission fluid.
There might be something wrong with the gears. Something may be jammed or stuck or they may just need changed out.
Just start it like normal and when the revs start to sound distorted start accelerating
There is a "dashpot" that works by vacuum, that revs the car as if you were giving it gas, like stepping on the old cars' pedal to keep it from cutting out when it cold-starts. All of the new cars have it.

I also wish someone would answer this question as my 1997 Ford contour does the same thing. It shakes and vibrates very bad when it idles and it idles really low. Seems to be a common fault with pumas. if your puma holds the revs after you've taken it out of gear it is most likely the throttle sensor (part costs £35). they had to run the diagnostics 3 times to figure this out, whereas ford only ran it once, charged £50 and said there was nothing wrong with it.
I would say your power brake booster is shot.
