More or less, yes. The new school needs to know what courses you have taken and passed, so they can credit you for those and place you appropriately.
First, most if not all the credits you completed will still be good, and will apply toward your degree. As far as the grades you received, they will still remain and be counted in as your grade point average (GPA), should you return to the same school. If you transfer to another school, they will transfer the credits, but not the grades. So, you start with a fresh GPA.

If the school was a public school, go to the main office of the system the school was in. They should be able to get your grades for you or tell you where to go to get them.
You don't write a letter to the teacher. He/she has nothing to do with the transfer. Go to the school office, get the forms for transfer, fill out the forms, and turn them into the school. The school will inform the teacher that the student is moving, to collect books, and to give the grades earned to the office. The office will send all forms needed to the new school.
Your grades in high school will not affect your admittance to law school. They will look at your college grades.
