Revelation 3:5 - He who overcomes shall be clothed in white garments, and I will not blot out his name from the Book of Life; but I will confess his name before My Father and before His angels. In other words, he who does NOT overcome, Jesus WILL blot out his name from the Book of Life. This conclusion is supported by statements such as that made in Revelation 2:5, where Jesus warns the entire church at Ephesus that He will remove their lampstand from its place unless they repent.

Presupposing you're some flavor of Catholic or Christian, and you're asking about a record of all persons who have ever lived created by God and maintained by God, the answer is: God as described in your religion is omnipotent. God can do anything God wants, to include writing, scribbling, annotating or erasing a name in said record. However, God would retain full awareness of this event, so if you're asking if the record of existence of a person could be erased from God's knowledge, the asnwer would be no. So: a record such as the one you describe would have to be for the use of beings other than God, such as saints, angels, demons, etc; because God would not need it. The existence of a person could therefore be erased from the awareness of every being except God.

Yes, in Rev. 3 :5 KJV states that if we do not turn from our wicked ways then He shall blot out our name from the Book of Life. You are alive, if you are reading this, and God deems it so. Yet if one dies in their sins, not having trusted in Christ, who paid for our sins, that name is blotted out of the Book of Life.

There are several verses in the last Book of Scripture that basically states what you are asking. Consider these two:

Revelation 22:19King James Version (KJV)

19 And if any man shall take away from the words of the book of this prophecy, God shall take away his part out of the book of life, and out of the holy city, and from the things which are written in this book.

Taking away 'his' part out of the book of life is akin to being 'removed' from the book. This verse is a bit closer to the wording of your question:

Revelation 3:5New King James Version (NKJV)

5 He who overcomes shall be clothed in white garments, and I will not blot out his name from the Book of Life; but I will confess his name before My Father and before His angels.

To 'not blot out' is same as not removing. Therefore, the answer is Yes, a name can be removed from the Book of Life.
