The first space shuttle, Columbia, was the first shuttle launched into space on April 12, 1981. Prior to this, the shuttle Enterprise had performed flights to test various capabilities of the shuttle within earth's atmosphere. Enterprise was not designed for space flight.
That partly depends on what you consider a moon landing. The first manmade object to sucessfully reach the lunar surface was the USSR's Luna 2, launched September 12, 1959, it impacted the moon's surface a few days later. The first soft landing on the moon was the USSR's Luna 9, which launched January 31, 1966 and sent TV panoramas and radiation data back to earth for 4 days.
The Apollo missions which reached the moon were launched from the Kennedy Space Center. The space shuttle was not designed to reach the moon, but instead orbits the Earth at 200-300 nautical miles.
None. The space shuttle came after the Mercury, Gemini and Apollo projects (Apollo got us to the moon.) The space shuttle was intended to be a freighter of sorts - ferrying materials, satellites and people from Earth's surface to low earth orbit (LEO) and possibly to one or more space stations in geosynchronous orbit. The shuttle was never intended for long distance flights to the moon or further**. They are to be retired next year (2010.) The first space shuttle launch was in 1981, 12 years after the first moon landing in 1969 and 9 years after the last manned trip to the moon. {If you use the official term for a moon landing, both manned and unmanned, then the Soviet Union landed an unmanned vehicle on the moon in 1966, 15 years before the first space shuttle launch.) ** Half of the shuttle (the black underbelly) is landing/shield tiles that burn off as it lands: if you were making a ship for space (and not our atmosphere) you wouldn't need these tiles, you'd need shielding but not the tiles.
In 1969 the shuttle didn't exist. No shuttle has landed on the moon to this date.

No space shuttle has been launched for the moon. It can't carry enough fuel to get there. Couldn't land if it got there.
The answer is none, the only astronauts who have been to the moon got there in various Lunar Modules, all of which were launched on Apollo rockets.

The Soviets launched a series of Luna probes that took the first pictures of the far side of the Moon. In 1966, Luna 9 made the first successful landing on the Moon and sent back television footage from the Moon's surface.
The first landing by man on the moon, was made at the Tranquility Base.
The first men walked on the moon after landing on it safely.
