Naruto is being badly beaten by Pain, and has been staked to the ground. Hinata jumped in to defend Naruto with the Gentle Step Twin Lion Fists jitsu. Hinata was thrown around pretty badly, and she was dying. Since she knew her time was short, she told Naruto how she had been feeling about him for all that time.

and i think it is obvious in the anime form the begining of part 2 she fell in love with naruto subconscious and she knows that she really lloves naruto when yamtao hint by that in episode 44 ...from th beginning from part 2 it is very obvious in the anime she loves naruto she always thinking about him and worried about him and never thinking in sasuke as naruto and when they was chasing sasuke after killing orotchimaro sakura was thinking in naruto and worried very mush about him while he was thinking in sasuke and why he never can hold him and it is alread satated in the anime that she moved on from sasuke and she is in love with naruto

EDIT: It was a fake confession to stop naruto from chasing after sasuke,it will mos likely end up as naruhina.Sakura still has feelings for sasuke.This is obvious,in the team 7 Reunion in the most recent manga,Sakura offered to go with sasuke again. yeah right befor she was about to stab sasuke with a posion knife and right after she came up with a plan to kill sasuke dude give all the details ok.
