This is true in the U.S. version of the movie. Van Cleef died last century after a long, colorful career in the movie industry.
However, in the Italian version of the movie, the title is a little different, and it might lead you to think that Eli Walsh (El torio) was the Bad.

The Good (Clint Eastwood), the Bad (Lee Van Cleef), and the Ugly (Eli Wallach) face off in a cemetery at the end of the movie. The Good kills the Bad as the Ugly realizes his gun isn't loaded. Then the Good forces the Ugly to stand atop a rickety wooden cross with a noose around his neck. As the Good rides away he shoots the noose, freeing the Ugly. The Ugly then realizes that the Good has left at least one bag of gold. So, although he lost the duel, at least the Ugly makes off with some reward for his troubles.
