Yes, She came back on The Forgotten Warrior. She is alive. She joined ThunderClan again.

No, as of the forgotten warrior she is alive. She ran into the tunnels but they didn't kill her. She went into hiding until Ivypool and Dovewing found her and Lionblaze brought her back to the clan. She is now a ThunderClan warrior.
