Most of it is all in their head, just like any other mental disorder. Even if they hit their goal weight, they can't just drop the dieting (which was bad in the first place) because of the fear they'll get fat. Its almost an addiction, and its hard to stop.The effects are both mental and physical, though.
They are considered mental disorders because usually someone who is anorexic hears a voice in there head that tells them they are fat and need to be skinny, that voice eventually becomes the persons only friend. Hope I helped
