Well actually Adrian left due to the bands heavy heavy touring schedule. That was the original impetus, he had been touring for almost ten years straight.
For the Powerslave tour, Maiden played 300 dates in 363 days. You have no idea what that kind of touring does to a person. PLUS he had a solo career he had been working on with his band Adrian Smith and Project, as well as Pyscho Motel(Hotel??).
But then he returned and all was well with the world again.
The second time he left, was an April Fools joke.
According to Adrian he lives "part time" in the US, but also lives in London.

There are two solos in The Number of the Beast. Dave Murray plays the first and the second is played by Adrian Smith.
If you mean a third guitar player it was because they wanted Adrian back but didn't want Janick to leave.
