It really depends on how you received the injury; however a bruise in the result of an insult to the skin. If the skin becomes abraded during the injury this would cause the scab to form. It is possible to have a bit of a scab from a bruise without noticing an abrasion to the skin and this has to do the the skin being porous and blood collecting near the surface.

Bruises do not disappear because they have no benefit from disappearing, or do not see this benefit. It's kinda the same question as: "Why does cancer not disappear?" They simply don't.

Red Blood Cells (RBC) do not "form a scab", they are merely trapped in the scab during blood coagulation. Platelets, carried in the blood serum form the scab by sticking to the endothelium (inside) of the blood vessel forming a plug to end bleeding. Clotting proteins than begin to condense and form the hard scab. Human RBC do not have DNA and therefore cannot act in response to external stimuli like a cut.
It depends. If the scab had just got finished bleeding, you have to wash it off, then when it fills, and if a bump is present, you can put ice on it.
