Explosions fall into two categories: deflagrations and detonations.
In a deflagration, the enthatlpy of reaction accelerates the rate of reaction until all the reactants are consumed. As the reaction occurs, it releases hot gasses which expand faster and faster as the reaction accelerates. To some degree the expanding gasses also compress the unreacted reactants ahead of the expanding shockwave - further accelerating the reaction but this is secondary to the effect of the heat. Deflagration type explosions have a sub-sonic wavefront.
In a detonation, the reaction front crosses the shockwave and compresses the reactants behind it. In a detonation, the exothermic front is thus "supersonic". Detonations are far more powerful than deflagrations with at least 4 times the maximum pressure (and often much more).
Deflagrations occur when there are sufficient reactants to sustain the reaction enough for pressure to build up and insufficient surrounding media to absorb the heat produced by the reaction. It is also important that the pressure be contained enough for the pressure to build up. Campfires usually do not explode because the rate of the reaction is not fast enough for the pressure from the expanding hot gasses of the flames to be contained - rather, they just push the surrounding air out of the way. Gunpowder poured out on the ground and ignited will just produce a quick flash, but not an explosion since the pressure is not contained and the amount of reactants is limited. If the reaction is contained and the pressure is allowed to build up and there is not enough surrounding air to absorb the heat being produced, an explosion may occur. Gasoline poured out on the ground will burn as a fairly steady flame, but the same gasoline when vaporized by the fuel injectors in a car, mixed with air, and then ignited in the cylinders, you get a short, contained deflagration that pushes the cylinder and moves the car. In a thermobaric (e.g. fuel-air) bomb, the fuel is dispersed through the air and then ignited in the center with the reaction accelerating through the dispersed cloud of fuel to create a major explosion. (Thermobaric weapons are especially effective in confined spaces like tunnels and caves.)
To summarize - explosions occur when there are sufficient reactants that can react fast enough that the air and other surroundings cannot move out of the way fast enough so that pressure builds up and the reaction keeps accelerating. Countermeasures include providing ways for the pressure to escape, starving the reaction of one of the reactants (in many cases this would be the oxygen in air), and providing something that will absorb the energy fast enough to slow the reaction down (excess air is a good heat sink for example).
