Fog appears on bathroom mirrors like how your hot breath appears on glass when you breathe on it. The hot vapor in the air mixes with cool glass and fog appears. In other words, water vapor condenses on a smooth, cold surface creating condensation (steam).

The warm water turns into vapor in the air because the pressure of the air is below the saturation pressure of the warm water at that temperature. The water evaporates. Then the water vapor moves throughout the bathroom and comes in contact with the mirror. The mirror is still at room temperature so it cools the water vapor down (past it's dew point temperature) which turns the vapor back into liquid water on the mirror.

The reason there is more water on the mirror is because glass and ceramics (the toilet) have a higher heat conductivity then other materials in the bathroom so it cools the vapor down and condenses it much faster.

Because it is a cold surface. When you bath you increase the amount of water vapor in the air in the bathroom and the cold surface of the mirror causes this dissolved water to come out of solution in the air and settle on the mirrors surface. Just like water forming on the outside of a glass full of ice.
