Fog forms on bathroom mirrors and car windows when the water vapor in the air condenses on the glass. This is due to the temperature difference between the glass and the air.
Due to the difference in the temperature, the molecules form a layer on the mirror ! That is what we call Fog !

Fog appears on bathroom mirrors like how your hot breath appears on glass when you breathe on it. The hot vapor in the air mixes with cool glass and fog appears. In other words, water vapor condenses on a smooth, cold surface creating condensation (steam).
When the windows begin to fog, the physical change is: condensation of water vapor.
You could exhale onto a mirror. The moisture in your breathe condenses on the mirror to form the "fog" that you see. Be quick though because the condensations evaporates very quickly. You can get a similar effect by taking a very warm shower, with nowhere to escape, the steam condenses on the mirrors or tile i your bathroom, and makes it moist or "foggy".

Hundreds of manufacturers produce and sell fog-resistant mirrors which can sometimes even be found in supermarkets.
There are good quality no fog mirror that indeed work. You can find great ones for a good price at your local Bed Bath and Beyond.
Zadro sells several fog resistant mirriors, along with spray solutions you can apply to your mirrior to make them more fog resistant by repelling water.
Fog can form during summer, depending on where you are. It depends on where you are located geographically. You may be in a location where fog forms very rarely.
To make an anti fog solution for mirrors mix together a 50-50 water and white vinegar solution. Add in a drop of dish soap and mix well. Spray the mirror and wipe it dry. That will last for a few days before you have to do it again.
