If you are playing GTA IV on a games disk, then it could be that the disk is At Fault. The first two GTA IV games disks I had were faulty like this, but I changed them each time, and the last disk I had is absolutely fine. If you can play other games on your console and they are OK, then it would point to this. You could also ask a friend who has the same game to loan you their disk and then see how it plays on your console. If their copy is fine, then you would seem to have a disk problem.
