Is the speedometer working? The computer controlled transmissions go into a safety mode when the VSS is disconnected. That causes oil pressures to go high, making the shifts much harder.

Lack of transmission fluid.
Could be many things. Have your universal joints checked.

The "lockup torque converter" could be engaging too soon. Take it to a transmission shop to have it diagnosed.
your transmission wears after time, try getting a fluid change, use fluid made for older cars this should help slow down wear and make the jerk much less noticeable
i have a 95 es300 but it doesnt jerk when i change gears when i ride it jerks but i found out it was a motor mount front motor mount which will cost bout $112 to replace but if when you change gears it could be your transmission slipping that cant be goo but check your motor mounts first....
In automatic transmissions, there is constant pressure form the pump at idle. This means that when going from Neutral to reverse, that pressure is suddenly slammed onto the drive train of the vehicle, if the engine doesn't stall. This gives the slight jerk/movement.
