Antisera is given to patients because it contains specific antibodies to the tetanus toxin. By binding, antibody to toxin, the toxin is neutralized. Otherwise, the toxin will cause tetanic musscle contractions seen in patients. This antisera causes a much faster response than the toxoid that is used as a vaccination. In this senario, a innactive form of the tetanus toxin (which is a toxoid) is introduced to healthy individuals. This toxoid causes the body (specifically B-cells) to produce antibodies and most importantly memory cells. Individuals who have circulating levels of antibody are considered immune to tetanus because the body will combat the pathogen before it has a chance to produce toxins.

The CDC has issued standard recommendations for the anti tetanus vaccine. keep in mind there is a difference between the vaccine (TD) and the immunoglobulin (TIG). If the individual has had 3 or more vaccinations in the past, then you would give the vaccination only if they had not received their last vaccination within the past 10 years (for non tetanus prone wounds) or 5 years (for tetanus prone wounds. he immunoglobulin (TIG) is given only to individuals with tetanus prone wounds who have had less than 3 previous vaccinations (or unknown status). As I understand, there is no last required tetanus shot. The vaccinations for tetanus are administered every ten years after the age of 6 years old. Before the age of six years old, a tetanus and diptheria vaccination is given when one is an infant, I believe. Exception is when one experiences in particular a deep would. If this occurs, a tetanus vaccination is given as what is called post-exposure prophylaxis. What that means is that the vaccination for tetanus is given due to the possibility that, due to the wound, one may have been exposed to the bacteria that causes the disease of tetanus. After the tetanus vaccination is given, it may take up to two weeks for the formulation of antibodies to prevent acquiring the disease of tetanus.

Either can be given to a 3 year old. The anti tetanus serum should be given if there is reason to believe the child has not been immunized against tetanus before and has been exposed. If the child has been immunized, and there is reason to believe that they have been infected, the toxoid should be administered.
The current recommendation is to receive a tetanus shot every ten years.
