Urine is tested for REDUCING SUBSTANCES. Glucose, fructose, etc are reducing substances. This test is non-specific. How ever, there is another test "Osazone test" that can identify the difference. In some people with uncontrolled Diabetes, urine is tested for ketone bodies. Presence of ketone bodies in urine indicates that the patient is in trouble.
The body releases everything it does not metabolize in the urine. Diabetics have high levels of sugar in their system unless they are reguarly taking a dose of insulin. When those levels get high, sugar will be released in the urine.
Generally insulin helps in the conversion of glucose into glycogen which is then stored in the liver. In people who have diabetes insulin is not produced hence the excess glucose in the blood stream is removed by the kidneys in urine.
Normally, when blood passes through the kidneys, some sugar remains in the fluid which later becomes urine. If that amount of sugar is low, which it normally is, the body can reabsorb the sugar from this fluid before it is excreted as urine. When the blood sugar is high, such as with diabetes, there is too much sugar to be reabsorbed, so some passes into the urine.
Excessive urine is a symptom of High blood sugar. The body will pull fluids from the body to "wash" out the extra sugar in the blood. Usually in this case the individual will also be drinking a lot to replace the lost fluids.
All of the sugar in the blood is filtered by the glomerulus of the kidney and passes into the tubule. The kidney tubules then re-absorb the sugar back into the blood. The tubules are only able to absorb all of the sugar up to a concentration of about 180 mg/ dl (normal is up to about 100) so anything above that get left behind and comes out in the urine.
In Type 1 diabetes, the body does not produce the hormone, insulin to break down the glucose in the blood stream. Similarly, type 2 diabetes does not respond to insulin, and sugar again goes not simplified. Because the kidneys do not break down glucose, the non-digested glucose passes through the digestive system and urinary tract.
