There are several factors in the overdose of users.
Probably the most common is that the user believes their tolerance for the drug is much higher than it actually is. Tolerance levels raise over time for a specific drug, which means an increase in dose is required to get high. Even though one's tolerance is very high, there still is a point of overdosing. Someone who is used to taking 100mg per day of Oxycontin can overdose if they try taking 150mg before their tolerance has built to that peak.
Another common factor is the attempt to get higher. Someone who is used to taking a drug over time, requires more of it to get high (thus the tolerance input above). Also when they have taken this drug for quite some time, the high is not the same as when they first began even though they were taking lower levels. The person will overdose trying to achieve the high that they had the first couple of times they ever took the drug, because it will need more than what their tolerance level can handle.
A third factor could be the mixing of drugs. It is not uncommon for users of Oxycontin to mix it with Xanax or for users of Cocaine to use Xanax to calm the nervousness caused by Cocaine. Mixing an upper with a downer is usually not a good idea. Some people survive it, many others don't. Besides, in this case, Oxycontin already suppresses the central nervous system (CNS) by itself. Adding Xanax to that will decrease its function even more, and can inhibit the brain function to breathe. Mixing Cocaine and Xanax is like running your car at full throttle 100 mph and then suddenly throwing it into reverse. It's going to kill your transmission and your car, just like it will kill a person who does this in some cases.
Recovering addicts who seek to use Methadone, should never mix another drug with it. Methadone is a blocking agent for other opioids. Since the person is not getting high with the narcotic because they have already taken Methadone, they will continue to take more of the narcotic to get the high. Thus, they end up taking too much of the narcotic for their system to handle and overdose.
I want to say that most drug addicts know what drugs can't mix with other drugs. I've always believed, in this area, that this is true, and those who overdose from a fatal combination of drugs did it because they knew what they were doing and were hoping for the overdose. That is another common reason for overdosing. Drug addicts are not generally happy go-lucky people. There are some depression issues, probably some post traumatic stress related to their use. Thus, they take these deadly combinations or fatal measurement of drugs for an easy way out - suicide.

It depends how big the miscalculations for the drug, but children always take less then adults so I would say it would be more deadly for children to overdose. (Children have to take less of a drug for it to be an overdose.)
I believe Cetirizine IS an antihistamine. And it would take a pretty large amount to overdose. MAOI drugs should not be taken with an antihistamine though because it can cause serious drug interactions I hear. But to answer the question- YES it is possible to overdose and die on any drug if enough is taken.
