If it was real - imagine the tension?
The two men/woman would really be determine to knock out the other person and it would turn out like one of them street fights!
So that's why it is scripted. So know one gets DANGEROUSLY hurt. They'll still get hurt, maybe a broken bone or a pulled muscle but they won't get too hurt that it's life threatening- which would be the case if it was not fake.
