Yes, when the drum itself or the pads inside are worn out they just dont do anything, its possible in your case you need to adjust the rear bake preassure if they look new and you have repatedly replaced the front brakes. I suggest having a mechanic adjust them, its easy and fast but if you dont know how to do it your car wont brake evenly or the brakes will wear too fast.
Check if the emergency brake cable isn't slightly on. This could could prevent the brake linings from clearing the lip on the outside edge of the brake drum. This is a common problem.

Remove the rear wheel, and the drum will slide off, unless its been there for a while, then you may have to tap around the edges with a hammer, and be sure you dont have the e-brake set, or you wont get it off.

Ii might be the way the brakes was bled. The brakes has to be bled with the car running and in this order: right rear, left front, left rear, right front. The computer wont recognize any other way.

yes the brake caliper has to fit into the wheel a smaller wheel may fit the lugs but wont go over the disk and caliper (or drum)
USUALLY the fatter line is for the front disk brakes. I am assuming your vehicle is disk / drum combination. To really confirm just trace the lines and see where they go. Trace the easiest line(s) first. You wont have to trace the last line as its the last wheel you didnt trace it to.
