From what I'm seeing, you can take a refresher course, but it all depends on what state you live in. Most states will allow the regresher course, while some may make you take the training again.
Most states have different rules for this problem but in general you can usually take an EMT-Basic challenge refresher course to be re-certified. i was certified as an emt about 12 yrs ago, how do i get certified

First of all, a licence by it's very nature suggests an automatic entitlement to drive. What you are referring to is most probably a certificate. A certificate should have no expiry date. The date you were trained or last tested along with the machine details are the important pieces of information and they don't expire. Prospective employers may very well consider the certificate 'expired' if it does not comply with their refresher / test intervals. If you have not driven trucks for some time we would recommend that you sit a refresher/test to ensure you are still operating safely.
CCNA re-certification: Take the CCNA certification again. CCNP re-certification: Before taking the CCNP exam, you must pass the CCNA exam (and the CCNA certificate cannot expire), Otherwise, there is no CCNA certificate and CCNP certificate through CCNP! CCNP certification is valid for three years. To recertify, Must pass any one of the three subjects in the CCNP level examination within the validity period of the certificate, or pass the existing CCIE written examination. Since you have both certificates, re-certify CCNP with a higher level.
You need to take a 6-12 week course followed by the certification exam. Seehttp://www.allalliedhealthschools.com/faqs/cna for more info
