So Bilbo is 128 when they meet at Rivendell on the 24th of October. After the fall of Sauron, the coronation and marriage of Aragorn etc. etc. the four hobbits of the story return to Rivendell on the 21st of September, and spend Bilbo's 129th birthday there with him. After the Scouring and reconstruction of the Shire, Sam and Frodo meet Bilbo again on the way to the Grey Havens, on 22 September 3021. That was Bilbo's 131st birthday. So Bilbo was 131 when he sailed into the West.
