Captain James T. Kirk had many crewmates Spock, Montgomery "Scotty" Scott, Pavel Chekov, Leonard "Bones" McCoy, Nyota Uhura, Hikaru Sulu & more but the ones mentioned are his main crewmates.

There are multiple possible answers to this.

Assuming you mean "lieutenant" in the "second-in-command" sense as opposed to the actual rank (US grade O-3, for naval ranks), it would have been Commander (O-5) Spock.

The next person in the chain of command would have been Lieutenant Commander (O-4) Montgomery Scott. (Dr. Leonard "Bones" McCoy was also nominally a Lieutenant Commander, but this seems to have been a courtesy professional rank and he doesn't appear to have actually been in the formal command structure.)

Next in line was probably Lieutenant Hikaru Sulu, the only actual "lieutenant" to be mentioned by name in the original series (as far as I can recall). Pavel Chekov was an Ensign (O-1) in the series, but had been promoted to Lieutenant by the time of the movies.
