The toothbrush wasn't invented until the 15th century by the Chinese The first toothbrush was made of hairs of a Siberian wild boar attached to a bamboo stick. Until this was invented people chewed ( still do) on sticks taken from certain trees. The World Health org suggests using the Misiwak chewing stick made from the Arak or Peeler tree. All that is needed to do is peel the bark off the end of the stick and chew. The stick not only cleans but provides an antibiotic that will help the mouth and gums. So, not only can you do what they did in history, but it may help.

Medieval dental care was usually provided by surgeons. A surgeon was not the same as a physician, who was more highly educated. Surgeons also tended wounded soldiers or people with broken bones. (Surgeons and barbers were united into a single profession of barber-surgeons, but that did not happen until after the Middle Ages ended.)

I think most people had rotted teeth and they subsequently ate on their gums. False teeth came after the middle ages I think.

there was no dental care, and if you had a problematic tooth, you got drunk and then got someone to hit you in the face with a rock so that the tooth would come out.
