First, beware of internet rumors-- Fox News is NOT banned in Canada, and it is also not banned at the White House. If I had to offer an educated guess, I would say that Fox News is widely available from any North American cable provider that offers it (and most cable providers do). But there are a few foreign countries that regularly limit or ban American media outlets of all kinds, not just Fox. China has restricted various American media sites (including the New York Times) at one time or other, and so have Iran, Syria, and other countries run by dictators.
