then talk to prof. rowan and youll get the natinal pokedex from oak and the pokeradar from rowan. Then go and use it in diffrent routes and catch rare Pokemon. (ex: catch flaaffy on route 222)
if your playing platinum, do the same and youll be able to get the three legendarie bird Pokemon: Articuno, Zapdos and Moltres when you go into his house in eterna city.

Have fun catching Pokemon with the pokeradar! To catch Dusclops on Pokemon Diamond you need the Poke Radar item first. Once you have the Poke Radar go to Route 224 and use the Poke Radar there and Dusclops should eventually appear in the grass.

no you cannot get thePokemon modifier with the poke radar, you must get it with cheats or at events.
I don't know if you would call this rare, but if you want a ditto, you could find that with the poke radar. Just fly to canalave city, leave the exit (southeast). Walk a few steps east and you will see a patch of grass. step in it and use the poke radar. If one of the shaking pieces of grass are sparkling, go in it. It could be a ditto.
The Pokemon you can find in Acuity Lakefront using the Poke Radar on Pokemon Diamond is Snorunt. Snorunt can be evolved into Glalie at level 42 or Froslass if it's a female and you use a Dawn Stone on it.
