Melting points of materials are affected mainly by one thing and that is the purity. It is impossible to raise the melting point, but it is possible to depress the range, that is the point at which it will begin to melt.

This depends on the physical properties of what is melting. But the speed at which an item is heated does not affect the temperature at which it melts, unless it is heated in an instant by extreme heat(100s of 1,000s of degrees). This can cause a metal to become a plasma which is matter that is more dense than lead but acts as a liquid.

The greater the temperature, the greater the rate of melting, all things being equal. The rate of melting will ultimately depend on the rate of heat transfer, and the rate of heat transfer will be dependent on the temperature difference between the ice and the surrounding air.
