If the traffic light is not powered, you are to treat it as a all-way stop sign. I'm assuming that's what you mean by that.
If the traffic light is broken, likely there will be utility workers out there working to fix it and you follow their instructions if any.

A traffic light that just turned green, we call this green light "fresh", meaning it just turned green and it won't be turning yellow for a little while. Of course these times between from when it turns green then back to yellow and red again varies on the intersection.
To answer the question when you're approaching a green traffic light you should prepare to stop as that green light is "stale". You don't necessarily have to slow down just get ready to hit the brake.

This depends on where you live.
Some places it means a "protected or Advance" signal meaning other you have right of way over on coming traffic.
Other places it can mean the light is about to change to yellow.
Or even that the signal is "n flash" or broken so use caution while crossing the intersection.

There is no exact time simply because no 2 intersections are the same, there are factors that change this time such as: How much traffic goes through the intersection on one direction?

It changed lives as the death toll dropped, as the number of cars increased there had to be a effect method of controlling the roads. This is when traffic lights were implemented. They slow down your day for a minute. But, they save your life a thousand times. Without them the roads would be havoc.
