A fishy smell can be a flavor defect that is the result of fat oxidation. Oxidation can occur from exposure to light - sunlight and artificial. Cartons are supposed to be best at protecting the milk from the light as compared to glass or plastic bottles.

There may be some milk products that have been fortified with Omega-3 (fish oil). Some people can pick up a fishy flavor in those products.

Cows themselves are clean animals that don't have an unpleasant smell at all. They do have some smell, but it is not as noticeable as the smell of horse. You can usually tell when a paddock contains cows because cow droppings have a distinctive smell, but it is not unpleasant in the way that dog droppings are (it's essentially digested grass). If cattle are kept clean and their droppings are removed often they do not smell at all. Cows that smell bad are probably sick and a barn or cattle paddock that stinks is probably overcrowded and poorly cleaned.
