As a general rule Cara and Mady Gosselin do not dress the same. However, they go to a private school so they probably wear the same (or similar) clothing for school and dress in similar (but not identical) clothing for photo shoots etc. Mady and Cara (as well as the sextuplets) tend to wear coordinating outfits for photo shoots and when the family goes out. As Kate put it, it is so the family looks coordinated as a whole.
