When a pro pierces your tongue you go into a clean room sit on a medical table or dentist chair...you rinse your mouth with listerine (the original yellow kind) for one minute...the artist takes a pair of clamps, a barbell and hollow needle out of separate sterialized bags. The wear gloves and the artist ask you to stick out your tongue they pick a spot clamp it in place and then pierce from the top to the bottom or vise versa i think its a preference issue...then put the barbell in which is longer to accomodate swelling. Pretty easy...but if you get it done you should eat first or else you get pretty light headed.
I would say to go to a place that does not use the clamps because that makes is swell up way more and bleed. I got mine done and they didnt use the clamps, i didnt bleed at all.
Your tongue should maintain its pink color after you pierce it.

If you don't pierce your tongue wrong then what's the problem?
If there is a regular tattoo/piercing shop, they'll pierce your tongue.
They will pierce with a hollow needle and hold the mark with a clamp. The procedure is very straight forward.

Not physically
Nothing, it's a piercing not a symbol.
