The question is asking how are you/what's up. So, you would answer with how you're doing or what's happening.

You can answer with your mood. If you want to make a full sentence, you would say: Estoy _____.

bien (good)

mal (bad)

cansado/a (tired)

feliz (happy)

triste (sad)

enojado/a (mad)

etc.

you can then ask how the other person is doing simply by saying "y tú/usted?"
