Depending on how often you use it, you will need to descale it every few months. Make a solution of one part vinegar and 3 parts water. Run it through the machine. Then run two cycles of clean water. You can make a solution with more vinegar if there's quite a bit of mineral build-up, but keep in mind you will need to run more cycles of water to get the smell and taste out. EDIT: Never use vinegar in an espresso machine, it will damage the internal components. Only use a recommended espresso machine descaling solution if possible, though you can use citric acid as well. It depends more on what type of water you are using than on how often you are using the machine. Harder water, like spring or well water, will need to be descaled very frequently (at least monthly) and softer water will only need to be descaled a couple of times a year. Regular filtered tap water, depending on your municipalty, might be ok with 3-4 times per year. Also, this does not explain how to clean your machine, only how to descale it. There is a difference.
